The information we collect
We collect the personal and health information needed to provide psychological services to you. This includes your contact details, Medicare and concession details, referral information from your GP or other referrer, and the clinical records made during your care.
How your information is stored
Your records are held securely and are accessible only to the clinical and administrative staff who need them in order to provide your care and to administer your account.
When information is shared
Information is shared with your referring GP or other treating practitioners where this supports your care, and with a paying agency where that agency is funding your sessions. Information may also be disclosed where there is a serious risk to you or to another person, or where disclosure is required by law.
Accessing your records
You can request access to the personal information we hold about you, and ask for corrections where it is inaccurate. Please speak to your psychologist or contact reception.
